摘要：

机器人胃癌根治术的安全性与可行性已得到证实，但机器人手术系统在术野显露、消化道重建环节仍存在若干技术难点。为此，本文提出了一种改良的机器人胃癌根治术，旨在规范手术流程、提升手术质量、缩短手术时间。该术式从多个维度优化了手术技术，包括优化穿刺器布局、改善基于三角手术平面的淋巴结清扫术野显露，以及采用右侧助手辅助孔布局以利于全器械吻合，从而保障消化道重建的同质性并缩短手术时间。在不影响手术安全性的前提下，该术式可帮助术者更高效地完成手术。

Abstract：

The safety and feasibility of robotic gastrectomy for gastric cancer have been well established. Nevertheless, several technical challenges remain with the robotic surgical system in terms of surgical field exposure and digestive tract reconstruction. To address these issues, we developed a modified radical robotic gastrectomy approach in this study, aiming to standardize the surgical procedure, improve surgical quality, and reduce operative time. This approach optimizes surgical techniques in multiple dimensions, including optimized Trocar positioning, improved surgical exposure for lymphadenectomy based on a triangular surgical plane, and a right-sided assistant port layout that facilitates fully stapled anastomosis, which ensures the homogeneity of digestive tract reconstruction and shortens the operative duration. This approach enables surgeons to complete the operation more efficiently without compromising surgical safety.
