慢性萎缩性胃炎患者疾病症状和生活质量与恐惧疾病进展的相关性研究

摘要:

目的 探讨慢性萎缩性胃炎(CAG)患者疾病症状和生活质量与恐惧疾病进展的相关性及影响因素。方法 采用随机抽样法抽取2021年6月至12月于西京医院门诊就诊的GAG患者229例,应用症状评分量表、SF-36生活质量量表和恐惧疾病进展量表对患者进行调查分析。结果 CAG患者的反酸、嗳气和腹胀较为明显,而食欲缺乏、恶心、呕吐、便秘和腹泻症状则较轻,差异均有统计学意义(P<0.05)。CAG患者的生活质量评分均低于全国常模评分,生活质量比较低下。除总体健康以外,存在恐惧疾病进展的患者生活质量各维度评分低于不存在恐惧疾病进展的患者,差异有统计学意义(P<0.05)。且患者的年龄、居住地、文化程度、职业、胃癌家族史、婚姻状况和肠化生程度都是影响CAG患者生活质量的因素。患者恐惧疾病进展量表平均分为(30.76±8.82)分,1/3以上的患者存在严重的恐惧疾病进展情况,出现了严重的心理功能失调。这种功能失调可能与年龄、职业、胃癌家族史、婚姻状况和肠化生程度有关。进一步相关分析发现,CAG患者的疾病症状与恐惧疾病进展评分相关(P<0.05)。生活质量方面,健康变化、生理功能、身体疼痛、活力、情感职能、精神健康和社会功能均与恐惧疾病进展量表评分相关(P<0.05)。结论 部分CAG患者存在严重的疾病症状,30%以上的患者有严重的心理功能失调,对疾病的恐惧进展程度较高。且患者的生活质量较为低下。无论是生活质量还是恐惧疾病进展,都可能与年龄、职业、胃癌家族史、婚姻状况和肠化生程度等因素有关,并且证明患者的疾病症状、生活质量和恐惧疾病进展均具有相关性。

Abstract

Objective To investigate the factors influencing disease symptoms and quality of life in CAG patients with fear of disease progression and their correlation. Method A random sampling method was used to select 229 patients with chronic atrophic gastritis who visited the outpatient clinic of Xijing Hospital from June to December 2021 as study subjects. The patients were investigated and analyzed by applying the Symptom Score ScaleSF -36 Quality of Life Scale and Fear of Disease Progression Scale. Result Patients with CAG had more pronounced acid refluxbelching and bloatingwhile poor appetitenauseavomitingconstipation and diarrhea were less severe all with significant differences (P<0.05). Quality of life scores of CAG patients were lower than national normative scoresand quality of life was lower. Except for general healthpatients with fear of disease progression had significantly lower quality of life scores on all dimensions than those without fear of disease progression with significant differences (P<0.05). Moreoverpatients' ageresidenceeducationoccupationfamily history of gastric cancermarital statusand degree of intestinal chemosis were all factors that affected the quality of life of CAG patients. The mean score of patients' fear of disease progression scale was (30.76±8.82)and more than one-third of the patients had severe fear of disease progression and developed severe psychological dysfunctionwhich may be related to ageoccupationfamily history of gastric cancermarital statusand degree of intestinal chemosis. Further analysis of the correlations revealed that both disease symptoms and fear of disease progression scores of CAG patients were correlated with significant differences (P<0.05). In terms of quality of life health changes physical functioningphysical painvitalityemotional functioningmental healthand social functioning were also all correlated with fear of disease progression scoresalso with significant differences (P<0.05). Conclusion Some CAG patients had severe disease symptomsand more than 30% had severe psychological dysfunction and a higher level of fear of disease progression. And the quality of life of the patients was relatively low. Both quality of life and fear of disease progression may be related to factors such as ageoccupationfamily history of gastric cancermarital status and degree of intestinal chemosisand it was demonstrated that patients' disease symptomsquality of life and fear of disease progression were all correlated.
