摘要：

目的  探讨胃癌患者血清miR-138表达水平及其临床意义。方法  采用实时荧光定量PCR(Real time Quantitative PCR, qRT-PCR)检测60例胃癌患者和60例健康体检者血清miR-138表达水平,分析miR-138与胃癌临床病理参数之间的关系。利用受试者工作曲线(Receiver Operating characteristic curves, ROC)评估miR-138诊断胃癌的敏感性及特异性。结果  胃癌患者血清miR-138水平明显低于健康对照组,差异有统计学意义(P<0.01);miR-138在血清中的表达水平与胃癌的分期以及是否发生转移有关,差异有统计学意义(P<0.05),但与患者性别、年龄、分化程度和有无Hp感染无关(P>0.05);ROC曲线下区域面积(AUC)为0.981(95%CI=0.962-1.000),其诊断胃癌的截断值为3.67,诊断敏感性为88.3%、特异性是95.0%。结论  miR-138作为抑癌基因可以作为早期胃癌的诊断标记物,且其表达下降与胃癌的转移以及分期有关,有着较高的临床诊断价值。

Abstract：

Objective  To investigate the expression of miR-138 in the serum of gastric cancer patients and clinical value. Methods  The expression of miR-138 in serum of 60 patients with gastric cancer and 60 healthy people were detected by Real time QuantitativePCR (qRT-PCR); The relationship between miRNA-138 and clinicopathological parameters of gastric cancer was analyzed; Receiver Operating Characteristic Curves (ROC)was used to determine the diagnostic value of gastric cancer. Results  The level of serum miRNA-138 in patients with gastric cancer was significantly lower than in healthy controls (P<0.01); The expression level of miR-138 in serum was correlated with the stage and metastasis of gastric cancer (P<0.05), but not with gender, age, differentiation degree and Hp infection (P>0.05); The area under curve (AUC) was 0.981 (95%CI=0.962-1.000) and when the cut-off value was 3.67, the sensitivity and specificity were 88.3% and 95.0% respectively. Conclusion  MiR-138 for a tumor suppressor gene can be used as a diagnostic marker for early gastric cancer, and its expression is related to the metastasis and staging of gastric cancer, which has a high diagnostic value.
