Expression and clinical significance of HER2/neu, HIF-1α and ki-67 in gastric adenocarcinoma

摘要:
目的 探讨表皮生长因子受体2 (HER2)、缺氧诱导因子1α(HIF-1α)与ki-67在胃腺癌的表达及其临床意义。方法
利用免疫组织化学法(IHC)和荧光原位杂交技术(FISH)检测118例手术切除并经病理确诊的胃腺癌中HER2/neu、HIF-1α和ki-67表达水平,并对结果进行统计学分析。结果 HER2、HIF-1α和ki-67在胃腺癌中的阳性率分别为13.56%、44.91%、79.66%;HER2阳性与Lauren’s分型和淋巴结转移相关(P<0.05),与年龄、性别、肿瘤部位、浸润深度、TNM分期不相关(P>0.05);HIF-1α高表达与淋巴结转移和TNM分期有关(P<0.05),与性别、年龄、肿瘤部位、Lauren’s分型浸润深度无关(P>0.05);ki-67高表达与Lauren’s分型、浸润深度、淋巴结转移和TNM分期相关(P<0.05),与年龄、性别、肿瘤部位不相关(P>0.05);ki-67高表达与HER2阳性、HIF-1α阳性均呈正相关,HER2阳性与HIF-1α呈正相关(P<0.05)。结论 在缺氧环境下,HER2阳性的胃癌细胞可能有更高的增殖活性,HER2、HIF-1α、ki-67高表达的胃腺癌较低表达者有更差的生物学行为,可能通过某些通路共同促进胃癌的发生和发展,为未来寻找新靶点提供临床依据。

Abstract:
Objective To investigate the expression of epidermal
growth factor receptor 2 (HER2), hypoxia inducible factor-1α (HIF-1α) and ki-67
in gastric adenocarcinoma and its clinical significance. Methods Immunohistochemical (IHC) and fluorescence in
situ hybridization (FISH) were used to detect the expression of HER2/neu, HIF-1α and
ki-67 in 118 cases of gastric adenocarcinoma. Results The positive rates of HER2, HIF-1α and ki-67
in gastric adenocarcinoma were 13.56% 、44.91%、79.66%; Overexpression/amplification of HER2/neu was associated with
Lauren's classification and lymph node metastasis (P<0.05), but not with
age, sex, tumor location, depth of invasion and TNM stage (P>0.05). The high
expression of HIF-1α was related to lymph node metastasis and TNM stage (P<0.05),
but not to sex, age, tumor location, depth of invasion and Lauren's
classification (P>0.05). The high expression of ki-67 was correlated with
Lauren's classification, depth of invasion, lymph node metastasis and TNM stage
(P<0.05). There was no correlation between age, sex and tumor site (P>0.05).
The high expression of ki-67 was positively correlated with HER2 and HIF-1α,
and HER2 was positively correlated with HIF-1α (P<0.05). Conclusion In anoxic environment, HER2 positive gastric
cancer cells may have higher value-added activity. The expression of HER2, HIF-1α
and ki-67 in gastric adenocarcinoma has worse biological behavior than that in low
expression, which may promote the occurrence and development of gastric cancer
through some pathways. In the future, it may become a new target of targeted
therapy in gastric cancer.
