摘要：

近年来，围绕胃癌膜的解剖（膜结构及其解剖层次）的研究不断深入，为实施规范化胃癌手术提供了解剖学依据。基于胃癌膜的解剖理念开展手术，有助于立体化清晰界定手术层面，规范淋巴结清扫路径，减少术中出血和手术相关并发症，并可能改善患者的围手术期结局和远期预后。为推动胃癌膜的解剖理念在胃癌外科治疗中的规范应用，中国抗癌协会胃癌专业委员会胃癌膜解剖研究协作组牵头，组织国内胃癌外科及相关领域专家，围绕胃癌膜的解剖相关名词定义及系膜的解剖边界，查阅了大量资料并结合临床所见，进行了充分论证。经过多轮讨论和专家投票，最终针对胃癌膜的解剖定义归纳了7条推荐意见；针对膜的解剖分离层面提出了39条推荐意见。本共识首次以胃癌膜的解剖为依据，理清了胃癌手术淋巴结清扫的边界，为临床医师实施规范化的胃癌手术，尤其对进一步提高胃癌的手术质量、促进我国胃癌诊疗水平的整体提升提供了有益的参考。

Abstract： 

In recent years, in-depth research has been conducted on the membrane anatomy of gastric cancer (including membrane structures and their anatomical layers), providing an anatomical basis for the implementation of standardized gastric cancer surgery. Surgery based on the concept of gastric cancer membrane anatomy helps to clearly define the surgical plane in a three-dimensional manner, standardize the route of lymph node dissection, reduce intraoperative bleeding and surgery-related complications, and may improve patients' perioperative outcomes and long-term prognosis. To promote the standardized application of the concept of gastric cancer membrane anatomy in the surgical treatment of gastric cancer, the Membrane Anatomy Study Cooperative Group of the Gastric Cancer Committee of the China Anti-Cancer Association took the lead in organizing domestic experts in gastric cancer surgery and related fields. Extensive literature review combined with clinical observations was performed to fully demonstrate the definitions of terms related to gastric cancer membrane anatomy and the anatomical boundaries of the mesentery. After multiple rounds of discussion and expert voting, 7 recommendations were summarized for the anatomical definition of gastric cancer membranes, and 39 recommendations were proposed for the anatomical dissection planes of membranes. As the first consensus based on gastric cancer membrane anatomy, this document clarifies the boundaries of lymph node dissection in gastric cancer surgery. It provides a useful reference for clinicians to perform standardized gastric cancer surgery, especially for further improving the quality of gastric cancer surgery and promoting the overall advancement of the diagnosis and treatment of gastric cancer in China.
